Note: Please send us the following information about your gift to enable us to issue Acknowledgment for your tax deduction purposes. You may inform us either by email, by fax or by mail whichever is convenient to you as mentioned below:

• Donor's full name and address
• Date of transfer
• Name and number of securities transferred
• Name of your brokerage house  
• Specific Purpose to which the gift should be directed, (if any)
